Use the simple form below to request medication request. You must be a permanently registered patient to use this service. Patients must collect their Prescription from their chosen Pharmacy.

Please allow at least 2 working days for us to process your prescription request. The pharmacy will need another 2 working days to dispense your medication. So allow at least 4 working days in total to collect your medication from the date you submit your request.
